Key Responsibilities:
Deliver direct patient care in line with a detailed care plan within the home environment
Maintain accurate and timely records of patient services
Educate patients and their families/caregivers effectively
Collaborate in the coordination of home care services
Engage in professional development and quality improvement initiatives
Ensure adherence to all state and federal regulations
Additional duties as assigned

Required Qualifications:
Completion of an accredited practical or vocational nursing program with valid licensure in the state of practice
Minimum of 1 year of nursing experience required, with 3+ years preferred
Experience in community health, hospice, or medical/surgical settings preferred
Strong commitment to clinical excellence and accurate documentation
