We are seeking a skilled Occupational Therapist to join our team at Alternate Solutions Health Network. As a Home Health Occupational Therapist, you will provide high-quality care to patients in their homes, helping them to regain independence and improve their quality of life.

• Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
• Develop and implement individualized plans of care to meet the unique needs of each patient.
• Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.

• Current license to practice as an Occupational Therapist in the state of employment.
• Minimum one year of experience as an OT in an acute care setting, with home care experience preferred.
• Valid driver's license and auto insurance.
• Ability to meet the physical demands of the job.
