Graves Gilbert Clinic is seeking a skilled Patient Care Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing direct patient care within the practice and directive of collaborating physicians. Your duties will include assessment and management of patients, day-to-day implementation and evaluation of the appropriate plan of care, and serving as a consultant, researcher, and educator.

You will work in collaboration with physicians and other healthcare professionals to provide patient-focused care. This will involve performing history and physicals, documenting findings, and communicating with collaborating providers. You will also be responsible for ordering appropriate diagnostic testing, reviewing results, and informing patients and providers of findings and required follow-up.

In addition to providing direct patient care, you will also be responsible for patient education and support, ensuring continuity of care for follow-up treatments, and adhering to HIPAA/OSHA/BLOODBORNE PATHOGEN regulations and maintaining associated workplace trainings.
