Physical Therapist

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities.
Key Responsibilities- Plan and administer prescribed physical therapy treatment and training for patients to attain the highest level of physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and develop treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment.
- Confer with physicians and clinical team members to obtain additional patient information and develop treatment programs.
- Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides.
- Monitor the completion of documentation by Physical Therapy Assistants and Home Health Aides.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations.
- Degree from an accredited Physical Therapy Program.
- Minimum of one year physical therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted Physical Therapy licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
