This opportunity offers flexible scheduling, with a requirement of being available 2-3 days per week. You will be working closely with a designated team to provide patient-centered care, making home visits, administering medication, wound care, treatments, and procedures as needed. You will also be responsible for monitoring clients' conditions, reporting changes to Clinical or Client Services Manager, following up with doctors' orders, and accurately documenting observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
