Job Description

At Sun Health, we are seeking a dedicated and compassionate Health Services Nurse to join our team. As a Health Services Nurse, you will play a vital role in providing exceptional care to our residents, ensuring their health and well-being are met with the highest level of professionalism and respect.

Key Responsibilities:

  • Assist the Emergency Medical Technicians (EMT) as needed to prepare and send Resident updates daily or completes as needed.
  • Attend Health Care Committee care plan meetings and facility utilization meetings as scheduled.
  • Advise and counsel residents on medical/health issues.
  • Correspond with family members, legal representatives, or physician when a resident displays behavior or health problems.
  • Elicit his/her involvement in making changes appropriate for treatment in the resident's behalf.
  • Assist in coordinating resident move to other facilities on and off campus when another level of care is necessary.
  • Accountable for all medical office expenditures.
  • Supervise and assist with monthly blood pressure clinics.
  • Visit individual apartments when appropriate.
  • Administer TB tests and Hepatitis B Vaccines for all employees and volunteers as needed.
  • Assist the director in maintaining and managing the EMT staff's schedule Bi-weekly.
  • Market and supervise the Companion program.
  • Check the status of discharged residents from the hospital within 24 hours upon return to the facility.
  • Assess and arrange for necessary assistance to meet residents' needs.
  • Communicate with hospital staff on condition of hospitalized facility residents.
  • Report to Health Care team and other staff members the potential return of the resident to the facility.
  • Maintain accurate resident files including initial and yearly profile updates.
  • Log and note incident reports.
  • Confer, when necessary, with outside agencies that are providing care for the resident.
  • Maintain and update resident Profiles in the computerized database.
  • Attend all mandatory meetings and in-services.
  • Assist the Director of Resident / Health Services with the day-to-day operations.

Qualifications:

  • Must be an L.P.N. or R.N. with a valid Arizona nursing license.
  • Possess a current Level 1 Fingerprint Clearance Card or can obtain is required.
  • Two years of experience working with seniors or related field.
  • Must maintain educational requirements and licenses necessary to perform in present capacity including CPR certification.
