We are seeking a compassionate and skilled Home Health Aide / Certified Nursing Assistant to join our team at LHC Group. As a Home Health Aide, you will play a vital role in providing personalized care and support to patients in their homes, promoting their physical and emotional well-being.
Key Responsibilities- Provide patients with in-home personal care and designated health-related services to maintain their physical and emotional well-being.
- Follow the written plan of care, Medicare/Medicaid regulations, and agency policies and procedures.
- Perform incidental household services essential to the patient's health care at home, such as mobility transfer, walking, grooming, bathing, dressing, eating, or toileting.
- Complete clinical notes for each visit within required timeframes and incorporate them into the patient's record.
- Successful completion of a competency evaluation.
- Current CPR certification required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Mature and able to deal effectively with the demands of a home health aide position.
