At Home Field Nurse

We are seeking a skilled and compassionate RN Care Manager to join our team at Providence Medical Group. As a Primary Coordinator of the Elder at Home care team, you will provide across-the-continuum, customized care coordination services and management of transitions of care for our most medically fragile patients and their families/caregivers.
Key Responsibilities- Provide patient-centered interventions and patient engagement strategies to promote self-management in the home/community setting.
- Visit patients in their homes to reduce ED utilization and hospitalization, and provide IV fluids, IV medications, EKG, and assessments as needed.
- Collaborate with our provider group of eight physicians to ensure seamless care coordination.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care.
- Oregon Registered Nurse License upon hire.
- 2 years Clinical medical-surgical, acute care or long-term care experience, including 1 year in community-based/ambulatory care and 1 year in care of the frail elderly.
