We are seeking a skilled Physical Therapist to join our team at Louis A Weiss Memorial Medical Center, LLC. As a Physical Therapist, you will be responsible for developing and implementing individualized physical therapy programs to improve and maximize patient mobility and independent functioning.
Key Responsibilities- Evaluate patients' functional abilities and disabilities, including range of motion and muscle testing.
- Establish treatment plans that specify objective treatment goals and modalities to assist patients in regaining function, increasing strength, and decreasing pain.
- Modify treatment plans as patients' conditions change.
- Perform physical, electrical, mechanical, and heat treatments, including facilitation, strengthening, and mobilization exercises, gait training, prosthetic training, massage, cervical and pelvic traction, ultrasound, hot pack application, Jobst RX, transcutaneous electrical neural stimulation, whirlpool, pulsed lavage, iontophoresis, biofeedback, and training patients with DL/adaptive equipment and in home exercise programs.
- Supervise treatments provided by physical therapist assistants and review treatment plans and patient progress with fellow treatment team members.
- Consult with social services/case management, home health agencies, and patients' families regarding discharge planning, including home treatment regimens, physical levels of independence, and nursing care needs.
- Document patients' treatment and progress in their charts, indicating their condition in relation to established medical goals.
- Current state of Illinois licensure as a physical therapist.
- Bachelor's Degree or certificate in Physical Therapy from an American Physical Therapy Association-approved school.
- Interpersonal skills necessary to communicate with patients and all levels of hospital personnel.
- Analytical ability required to evaluate patients' progress and response to treatment and plan and adjust treatment plans.
- Ability to effectively and accurately operate all department equipment related to physical therapy treatments.
