Job Title: LPN/LVN - LTC/SNF Healthcare Professional

We are seeking a compassionate and organized LPN/LVN to join our team at 603Healthcare - Mineral Springs. As a key member of our healthcare team, you will provide exceptional care to our patients and make a significant impact in the healthcare community.

Job Summary:

As an LPN/LVN, you will be responsible for delivering high-quality patient care in a long-term care/skilled nursing facility setting. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and providing emotional support to patients and their families.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Maintain accurate and up-to-date patient records, including progress notes, medication lists, and other relevant information.
Requirements:
  • Current LPN/LVN license in the state of New Hampshire.
  • At least two years of recent experience as an LPN/LVN in a long-term care/skilled nursing facility setting.
  • Current BLS and/or ACLS and/or specialty certifications.
  • Two current clinical references.
  • Physical and TB records.
  • Current Tdap/Flu vaccinations.
  • Titers - MMR/Hep B/Varicella or vaccination records.
  • Ability to pass background/urine drug screening upon accepting an offer.
