Job Description

Emblem Home Health is seeking a skilled Physical Therapist to join our dynamic team. As a Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes.

About the Role

We are looking for an experienced and motivated Physical Therapist to work on weekends as a home health physical therapist. The successful candidate will be responsible for assessing and evaluating patients' therapeutic, rehabilitative, and functional status and directing physical therapy treatment according to physician's plan of care.

Responsibilities
  • Conduct thorough assessments and develop individualized care plans in partnership with patients, representatives, and caregivers.
  • Provide education and counseling to patients, families, and caregivers on therapeutic appliances and techniques.
  • Prepare and maintain accurate clinical notes and progress summaries based on goal attainment.
  • Communicate effectively with physicians and other healthcare practitioners regarding patients' current home health plans of care.
  • Deliver high-quality physical therapy services, including exercise programs, stretching, and strengthening exercises, to promote patient mobility and independence.
  • Instruct patients, families, and caregivers in the safe use and care of equipment, such as walkers, canes, and wheelchairs.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that address patients' medical, social, and emotional needs.
Requirements
  • Licensed Physical Therapist with a degree from a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited program.
  • At least two years of experience as a Physical Therapist, preferably in community or home health settings.
  • Demonstrated ability to communicate effectively with patients, families, and healthcare professionals.
  • Possession of current CPR certification.
  • Valid driver's license with reliable transportation.
