AdaptHealth LLC is seeking a skilled Nurse Specialist to provide specialized nursing services to patients and caregivers at home or in alternative settings.
The ideal candidate will have a strong background in clinical nursing and experience working with patients in home health care settings.
Key Responsibilities:
- Provide technical nursing and skilled interventions as ordered by the physician.
- Instruct, motivate, and assist patients/caregivers in learning and improving ability to live independently at home.
- Ensure physician orders are followed and included in patients plan of care.
- Continually monitor patient outcomes and report changes to supervisors to prevent hospital readmission.
- Participate in effective communication amongst other clinicians providing care to the patient.
- Complete and submit all required documentation within established agency guidelines.
- Communicate the patient's progress to supervisor and make changes as necessary under the physician's direction.
- Cover after hours, weekend, and holiday on call to meet the branch's need.
- Maintain confidentiality of all company and patient-related information.
- Protect patient rights as defined in the patient Bill of Rights.
- Attend and participate in staff meetings, required in-services, and other meetings as requested.
- Ensure accurate patient assessments are completed timely.
- Coordinate related services for patients.
- Respond to emergency patient needs timely and escalating to supervisors and physicians as needed.
- Represent self in a professional and ethical manner.
- Take professional responsibility for self-protection during required work in potentially hazardous situations.
- Complete all required patient care documentation and reports accurately and in a timely manner.
- Communicate with clinical staff, physicians, and other healthcare providers to assure the quality and continuity of patient care.
Requirements:
- Associate degree in nursing or equivalent.
- Current and unrestricted LPN clinical license.
- Two (2) years of clinical nursing experience is required.
- Five (5) years of related HME experience is preferred.
- Valid and unrestricted driver's license.
