About the Role
As a vital member of our Long Term Acute Care team, you will play a key role in transforming healthcare through innovation, compassion, and a steadfast commitment to delivering exceptional patient care.
Position Details:
SHIFT: 7 PM - 7:30 AM / FULL-TIME / 36 HOURS
LOCATION: Emory Long Term Acute Care Hospital
As a skilled clinician, you will leverage your extensive experience in patient care, demonstrating a profound understanding of the specific needs of your patient population. You will:
- Utilize your knowledge, theory, research, and experience to adapt to evolving clinical situations.
- Implement the Synergy Model for Care to tailor care plans for each patient and their family.
- Act as a resource for colleagues and advocate for patients and families.
- Communicate effectively and respectfully with patients, families, and the healthcare team.
- Deliver safe, reliable, and effective care independently, applying the nursing process.
- Exhibit strong assessment skills and recognize the influence of nursing care on patient outcomes.
- Document the nursing process accurately and in the appropriate format.
- Plan and execute patient care autonomously, incorporating patient and family input.
- Evaluate patient responses to care and adjust plans as necessary.
- Identify learning needs of patients and families, recommending resources as needed.
- Incorporate the AACN Synergy Model into your clinical practice.
- Manage time efficiently and recognize the need for policy revisions based on evidence-based research.
- Delegate tasks appropriately while utilizing the Synergy model of care.
- Understand the roles of the interdisciplinary team and utilize them effectively in patient care planning.
- Take on leadership responsibilities as assigned, including Charge Nurse duties.
- Orient new staff or students in the absence of a designated preceptor.
- Utilize resources responsibly with a focus on cost containment.
- Demonstrate effective listening and conflict resolution skills.
- Individualize care based on knowledge of patients and their families.
- Recognize and advocate for patient needs based on their conditions.
- Maintain confidentiality and accept accountability for professional growth.
- Participate in unit initiatives to address discipline-specific issues.
- Articulate the application of the Synergy model in clinical practice, collecting and analyzing data as required.
Minimum Qualifications:
Graduate of an accredited nursing program with at least 12 months of recent nursing experience within the past five years or recent completion of a re-entry nursing program. A valid, active, unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board is required. BLS certification is mandatory, and ACLS may be necessary in designated departments.
