Job Title: Certified Nursing Assistant - Long Term Care - Skilled Nursing Facility

We are seeking a compassionate and organized Certified Nursing Assistant to join our team at Meadowbrook at Black River Falls. As a CNA, you will play a vital role in providing exceptional care to our residents.

Job Summary:

The ideal candidate will have a current Wisconsin license/certification and at least two years of recent experience as a CNA in Long Term Care - Skilled Nursing Facility. You will be responsible for delivering high-quality care, maintaining a safe and clean environment, and collaborating with our interdisciplinary team.

Responsibilities:
  • Provide direct patient care, including bathing, dressing, and feeding
  • Assist with daily living activities, such as toileting and ambulation
  • Monitor and report any changes in patient condition
  • Participate in care planning and implementation
  • Maintain accurate and detailed records
Requirements:
  • Current Wisconsin license/certification
  • At least two years of recent experience as a CNA in Long Term Care - Skilled Nursing Facility
  • Current BLS and/or ACLS and/or Specialty Certifications
  • 2 current Clinical References
  • Physical
  • TB Records
  • Current - Tdap/Flu vaccinations
  • Titers - MMR/Hep B/Varicella or vaccination records
  • Must be able to pass background/urine drug screening upon accepting an offer
