Position Summary:
The Hospice Registered Nurse will oversee and coordinate care for assigned patients in a hospice setting, providing expert palliative care. This role requires exceptional clinical, observational, and interpersonal skills, along with sound nursing judgment. The nurse will collaborate closely with a dedicated hospice team to ensure optimal patient care and outcomes.
Key Responsibilities:
- Deliver direct care to patients in various environments, addressing all needs and goals.
- Conduct admission assessments, regular skilled nursing visits, and hospice evaluations.
- Maintain accurate and comprehensive patient medical records in line with agency standards.
- Demonstrate effective teamwork and communication with the interdisciplinary team.
- Participate in case presentations during team meetings and contribute to educational programs.
- Manage patient care with a focus on pain and symptom management, grief, and loss.
- Conduct supervisory visits and reassess nursing needs of patients.
- Provide timely and complete documentation of patient interactions and care provided.
- Engage in case coordination meetings with healthcare partners to ensure seamless service delivery.
- Respond to emergencies and report changes in patient conditions promptly.
Qualifications:
- Current Registered Nurse license in the State of Oklahoma.
- Minimum of two years of hospice experience preferred.
- Certification in Hospice and Palliative Nursing is advantageous.
- Strong verbal and written communication skills.
- Ability to adapt to changing tasks and maintain focus amid interruptions.
Physical Requirements:
- Ability to perform light to moderate physical activities, including lifting up to 50 lbs.
- Good color vision and hearing are essential for patient assessment.
