Key Responsibilities- Provide direct patient care to patients with a variety of physical and/or behavioral problems.
- Work collaboratively with the Registered Nurse (RN) and other healthcare professionals to develop and implement individualized care plans.
- Administer medications and treatments as prescribed by the RN or physician.
- Monitor patients' conditions and report any changes or concerns to the RN or physician.
- Participate in patient education and counseling to promote healthy behaviors and disease management.
- Collaborate with other healthcare professionals to ensure seamless care transitions and continuity of care.
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
- Current licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare professionals.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Experience working in a clinical setting, preferably in a hospital or outpatient clinic.
- Knowledge of electronic medical records and computerized charting systems.
- Ability to work independently and as part of a team.
- Excellent problem-solving and critical thinking skills.
