Job Summary

Creekside Health and Rehabilitation is seeking a skilled and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our night shift team. As a nurse at our facility, you will play a vital role in providing high-quality care to our residents and making a meaningful difference in their lives.

Key Responsibilities
  • Provide daily care, health needs, safety, and comfort to residents, including assessments, documentation, and management of nursing units and staff.
  • Develop and implement resident care plans, maintaining up-to-date nursing care plans, documenting progress, and making recommendations for revisions.
  • Make daily and periodic resident visits to monitor and observe physical and mental conditions, performing routine nursing procedures as needed.
  • Work closely with physicians and families to maintain residents' health and mental state, providing emotional support and handling resident and visitor complaints or incidents.
  • Document medical records, complete 24-hour reports, end-of-shift reports, shift narcotic counts, and document the destruction of outdated/discontinued medications.
  • Maintain up-to-date resident medical records.
Requirements
  • Graduate of an accredited school of nursing and possess a valid RN/LPN license in good standing according to State and Federal requirements.
  • Thorough understanding of safe and effective nursing practices within the State of Indiana.
  • Basic computer knowledge and skills.
  • Passion for caring for people.
  • Personal integrity and ability to deal tactfully with residents, family members, visitors, government agencies, and other personnel.
  • Ability to maintain confidentiality and adhere to Corporate Compliance and HIPAA guidelines.
