About the RoleJob Overview

This position involves working with clients in their homes, following established care plans, and providing personalized support with activities of daily living.

Key Responsibilities:
  1. Implement client care plans, ensuring continuity and quality of care
  2. Assist clients with bathing, showering, and partial bathing as needed
  3. Support clients with maintaining personal hygiene and daily living skills
  4. Provide education and guidance to clients and family members on self-care techniques
  5. Accompany clients to doctor appointments and provide assistance as needed
  6. Monitor and record client vital signs, including blood pressure, temperature, and respiration
Qualifications:
  • Certified Nursing Assistant (CNA) certification highly desirable
  • Active registration on the Nurse Aide Registry in the state of employment required
  • At least 1 year of experience in a home health or hospice setting
  • Minimum 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility)
  • Excellent communication and interpersonal skills, with ability to build strong relationships with clients and colleagues
