Key Responsibilities:- Provide holistic, high-touch, and coordinated care for a panel of senior patients
- Deliver outstanding clinical care and support functions related to patient care activities
- Actively engage and manage patient panels in collaboration with the Nurse Practitioner
- Ensure efficient clinic flow by rooming patients promptly and assisting the provider in maintaining a timely schedule
- Support post-visit activities, including facilitating referral completion and conducting follow-up outreach to patients as needed
- Promote clinical and safety standards to drive high-quality care for our patients
- Maintain a safe, secure, and healthy work environment by establishing and following standards and procedures
- High School Diploma or GED equivalent
- Graduate of an accredited Licensed Practical Nursing program
- Valid LPN license
- 3+ years of experience as a Licensed Practical Nurse
- Primary Care experience is a plus
- 1+ year of Phlebotomy experience (preferred)
- Current BLS certification endorsed by the American Heart Association (Preferred)
- 6+ months of laboratory experience (Preferred)
