About the Role
As a Social Worker with Providence, you will play a vital role in evaluating patient and family psychosocial and environmental needs, developing and implementing social work treatment plans, and documenting patient care consistent with insurance guidelines.
Our Social Workers work closely with Home Health Managers to ensure high-quality patient care and outcomes. We value our caregivers and strive to create a culture of patient-focused, whole-person care built on understanding, commitment, and mutual respect.
Responsibilities
- Evaluate patient and family psychosocial and environmental needs
- Develop and implement social work treatment plans
- Document patient care consistent with insurance guidelines
- Collaborate with Home Health Managers to ensure high-quality patient care and outcomes
Requirements
- Master's Degree in Social Work (MSW) from a school of Social Work
- Washington Advanced Social Worker License or Associate License
- 1 year of experience in a healthcare setting
- 3 years of post-master's work experience or prior experience in hospice care
Preferred Qualifications
- 2 years of experience in a healthcare setting
- Prior experience in home health or hospice care
