Avera Oahe Manor is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in delivering high-quality patient care in our long-term care setting.
**About the Role:**
As a Licensed Practical Nurse, you will be responsible for providing direct patient care services, including assessing, planning, evaluating, and teaching patients and their families. You will work collaboratively with our interdisciplinary team to ensure that our patients receive comprehensive and compassionate care.
**Responsibilities:**
* Provide direct patient care services, including administering medications, performing treatments, and providing wound care
* Assess patients' physical and emotional needs, and develop individualized care plans
* Collaborate with the healthcare team to ensure that patients receive comprehensive care
* Educate patients and their families on health promotion, disease prevention, and management
* Participate in quality improvement initiatives to enhance patient care
**Requirements:**
* Graduate from an accredited school of nursing
* Licensed as a LPN in the State of South Dakota
* Basic Life Support (BLS) certification
* 3-5 years of experience in long-term care
