Job Summary

We are seeking an experienced Home Health Registered Nurse to join our team in a Travel Nursing position. As a Home Health Registered Nurse, you will be responsible for providing high-quality patient care and support to individuals in their homes. This is a fantastic opportunity to work with a dynamic and supportive team, and to make a real difference in the lives of our patients.

Responsibilities

  • Provide comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient.
  • Deliver high-quality patient care, including medication management, wound care, and education on disease management and self-care.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.

Requirements

  • Current RN licensure in the state of practice.
  • Minimum of 1 year of experience in Home Health or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
