We are seeking a highly skilled and compassionate RN Case Manager to join our team at Benedictine. As a key member of our nursing team, you will be responsible for providing exceptional patient care and leadership to our team of nurses.
Key Responsibilities- Conduct Comprehensive Assessments: Evaluate patients' physical, emotional, and social needs to develop individualized care plans.
- Develop and Implement Care Plans: Collaborate with patients, families, and healthcare professionals to create and adjust care plans as needed.
- Coordinate Care Services: Work with various healthcare providers to ensure seamless care delivery and effective communication.
- Patient Education and Advocacy: Educate patients and families about their conditions, treatment options, and self-care techniques, while advocating for their needs and preferences.
- Quality Improvement and Compliance: Maintain accurate records, ensure quality care delivery, and participate in quality improvement initiatives.
- Current State Licensure: Must hold current state licensure as a RN in good standing.
- Leadership Experience: Possess supervisor/management experience and excellent communication skills.
- Organizational Skills: Demonstrate organization skills required to assess, plan, implement, and evaluate resident care.
- Language Skills: Be able to read, write, and communicate in English.
