We are seeking a highly skilled RN Clinical Manager to join our team at CenterWell Home Health. As a Clinical Manager, you will be responsible for overseeing the delivery of high-quality patient care and ensuring that our patients receive the best possible outcomes.
Key Responsibilities- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct assessments and reassessments of patients, including updating of care plans and interpreting patient needs.
- Manage the assignment of caregivers and oversee the delivery of care to all patients served by the location.
- Responsible for QA/PI activities, including data tracking for performance review and outcomes of care analysis.
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
- Home care experience is required, preferably home health.
- Management and people leadership experience is required.
- OASIS experience is required.
- OASIS certification (HCS-O, COQS, and/or COS-C) is highly desired.
- Homecare Homebase (HCHB) experience is required.
- CMS PDGM billing knowledge and/or experience is required.
- Competitive salary range: $76,800 - $105,800 per year.
- Annual bonus eligible.
- Normal hours of operation: M-F / 8a-5p (ET).
- On-call expectation: Yes, rotating on-call shift.
- Branch size: 460+ Census (3.5 STAR rating).
