**Wraparound Facilitator

About the Role:

Eckerd Connects is seeking a highly skilled and compassionate Wraparound Facilitator to join our team. As a key member of our Coordinated System of Care, you will play a vital role in providing individualized, coordinated, family-driven care to youth and families in need.

Key Responsibilities:

• Facilitate a high-fidelity wraparound process for youth and families, ensuring a comprehensive assessment and development of a personalized plan of care.
• Collaborate with families, providers, and key stakeholders to build a support network that addresses the complex needs of youth at risk of out-of-home placement.
• Work closely with Independent Evaluators to ensure that assessments are completed and results are incorporated into the care planning process.
• Develop and implement individualized plans of care that adhere to National Wraparound Initiative standards and treatment planning requirements.
• Assist youth and families in accessing strength-based mental health, social services, educational services, and other necessary supports and resources.

Requirements:

• Bachelor's degree in a social, human services, or behavioral sciences field (or equivalent experience in a relevant field).
• Previous experience working in a family, children/youth, or community service capacity (or equivalent experience in a relevant field).
• Ability to obtain a wraparound practitioner certificate within 2 years of hire date and maintain certification annually.
• Extensive travel and driving required; ability to provide 24-hour, 7-day on-call crisis intervention to youth.
• Flexibility to work a varied schedule, including nights and weekends, and ability to transport youth, volunteers, and others.
• Pass a drug screen in compliance with Eckerd Connects' Substance Abuse and Drug-Free Workplace policy.
