AleraCare, a renowned provider of infusion treatments and specialty pharmacy services, is seeking an experienced Ambulatory Infusion Nurse Practitioner to join our team at our San Jose infusion center. Our clinics operate weekdays from 8:00am-5:00pm, offering a stable and secure work environment.

Job Description:

The ideal candidate will possess excellent nursing skills, strong interpersonal abilities, and the capacity to provide high-quality patient care in accordance with professional standards and state regulations.

  • Responsibilities:
  • Maintain patient confidentiality at all times.
  • Function as a patient advocate, treating patients and families with respect and dignity.
  • Demonstrate competency in providing patient care and adhere to professional standards.
  • Perform initial patient evaluations/assessments and develop a written plan of care.
  • Prepare clinical and progress notes, coordinate services, and inform the physician and other personnel of changes in the patient's condition and needs.
  • Assure that progress reports are succinct when the patient's condition changes or deviates from the plan of care.
  • Regularly reassess the patient's condition, nursing needs, and update the plan of care.
  • Furnish those services requiring substantial and specialized nursing skills.
  • Initiate appropriate preventive nursing procedures.
  • Collaborate with physicians and other members of the healthcare team to ensure optimal patient care.
  • Evaluate and reevaluate the patient and/or family for changing need for services.
  • Provide patient and/or family with health education, counseling, and referrals to other healthcare professionals and/or community resources when appropriate.
  • Order and/or interpret lab work and diagnostic tests according to patient need and/or plan.
  • Demonstrate the ability to prescribe medications and non-pharmacological therapies based on the patient's diagnoses, health history, and age.
  • Initiate case conferences and communicate the patient plan of care with team members.
  • Participate in the company's Performance Improvement and Continuous Quality Improvement (CQI) programs.
  • Participate in committees when requested.
  • Attend continuing education programs and staff meetings as appropriate to job responsibilities.
  • Assume responsibility/ownership of clinic location(s).
  • Support and maintain a culture of safety and quality care.
  • Stay current with medical advancements, new technology, new drugs through inservices, continuing education classes, society meetings, professional journals, etc.
Requirements:
  • Education and Certification:
  • Master's Degree from an accredited Nurse Practitioner Program.
  • Holds a current NP state license in the state of practice.
  • At least 4-5 years of advanced practice registered nursing (APRN) experience, ambulatory and infusion nursing experience preferred.
  • Skills and Qualifications:
  • Strong interpersonal, time management, and organizational skills.
  • Strong written and verbal communication skills.
  • Experience with electronic medical records (EMR). Must have organizational skills to maintain and manage accurate patient care records.
  • Ability to recognize and direct information to the appropriate health care provider.
  • Computer skills that include word processing and efficient use of the internet and e-mail.
  • Current CPR card, advanced practice certification, current APRN licensure, valid driver's license, reliable transportation, automobile insurance.
Estimated Salary:

$120,000 - $150,000 per annum, depending on experience and qualifications.
