Key Responsibilities- Develop and Implement Case Management Plans: Collaborate with healthcare teams to create and execute individualized care plans that address patient needs and promote efficient transitions between care settings.
- Referral and Coordination: Identify high-risk patients and refer them to social services or other relevant resources to ensure timely and effective interventions.
- Transition of Care: Develop and implement discharge/transition plans to ensure seamless transitions between care settings and promote patient safety.
- Financial Management: Communicate with healthcare teams and patients/caregivers regarding financial management issues that may impact patient care.
- Patient Progression: Monitor patient length of stay and actively participate in care rounds to ensure efficient and effective transitions.
- Regulatory Compliance: Maintain knowledge of federal, state, and other regulatory agency rules and regulations, including The Joint Commission, CMS, and Medicare.
