Job Summary

We are seeking a highly skilled Advanced Practice Provider to join our multidisciplinary health care team in a primary care setting. As a key member of our team, you will provide comprehensive care to patients across the life span, working collaboratively with an interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities
  • Provide primary care to patients, including preventive health, acute and complex care visits, and referrals to specialists as needed.
  • Collaborate with the health care team to develop and implement individualized plans of care.
  • Conduct thorough patient assessments, including physical exams and diagnostic testing, and interpret results to inform treatment decisions.
  • Formulate and document plans of care, utilizing evidence-based practice guidelines and protocols.
  • Prescribe diagnostic strategies and therapeutic interventions, and perform in-office procedures as needed.
  • Document patient encounters in the EPIC system, meeting compliance standards and capturing relevant patient information.
  • Communicate effectively with patients, families, and the health care team to ensure seamless care coordination.
  • Participate in quality improvement initiatives and engage in ongoing education and professional development to enhance clinical skills and knowledge.
Requirements
  • Masters degree in a relevant field.
  • National Board Certification as a Primary/Family Care Nurse Practitioner or Physician Assistant.
  • BLS certification.
  • Covid vaccination required for employment.
