Hospice Caregiver

The Hospice Caregiver serves as a vital support role, delivering comprehensive personal assistance and addressing various healthcare needs for patients in accordance with a structured Care Plan.
KEY RESPONSIBILITIES:
- Provides or aids patients with personal hygiene tasks, including bathing, dressing, and oral care.
- Assists patients with toileting needs, utilizing appropriate equipment as necessary.
- Supports patients during meal times, including preparation of special dietary requirements.
- Facilitates patient mobility, including transfers and ambulation, with or without assistive devices.
- Guides patients through prescribed exercises and range of motion activities as instructed by healthcare professionals.
- Applies non-prescription topical treatments as needed.
- Monitors and records vital signs, including temperature, pulse, respiration, and blood pressure.
- Conducts routine catheter care and assists with catheter removal under professional guidance.
- Performs basic dressing changes and provides care for pressure sores.
- Assists with ostomy care, including pouch management and irrigation preparation.
- Documents care activities and observations in designated records.
- Monitors tracheostomy sites and reports any irregularities to nursing staff.
- Engages with patients' families and other healthcare personnel to ensure effective communication.
- Observes and reports any changes in patients' physical or mental conditions promptly to nursing staff.
- Delivers holistic support to hospice patients.
- Serves as a mentor for new staff members during their orientation.
PHYSICAL AND SENSORY REQUIREMENTS:
Must be capable of sitting for up to 2 hours daily and standing for up to 10 hours daily. Ability to walk for 6 hours daily is required, with occasional lifting of patients weighing up to 250 pounds using appropriate methods. Frequent bending, reaching, and manual dexterity are essential. Must possess normal hearing and vision capabilities.
EDUCATION:
A high school diploma or GED is mandatory. Completion of a training program that meets the standards set by the Department of Health and Human Services is required.
EXPERIENCE:
A minimum of two years of experience as a Home Health Aide under qualified supervision in a Medicare Certified program is required. Experience as a Certified Nurse Aide or Personal Care Aide is preferred. Proficiency in English communication is essential.
WORK SCHEDULE:
Availability for weekend shifts is required.
