Job SummaryWe are seeking a dedicated Physical Therapist to join our CarePartners Home Health team. As a Primary Physical Therapist, you will manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
Responsibilities- Completes comprehensive assessments, develops and modifies plans of care in conjunction with patients, caregivers, and physicians
- Provides treatment visits and discharge planning to facilitate patient engagement and achievement of goals, clinical outcomes, and positive patient experience
- Communicates handoff information and coordinates with physical therapists, physical therapist assistants, and other care team members to support patient care throughout home health and the continuum of care
- Bachelor of Science degree in Physical Therapy
- Current North Carolina license to practice Physical Therapy
- Preferred experience in clinical affiliations with a wide variety of acute care procedures, especially orthopedics, neurology, cardiopulmonary, and care of burns/open wounds
