Job Title: Licensed Practical Nurse - Community Care Specialist

Job Summary:

We are seeking a skilled Licensed Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to individuals with complex medical needs, developmental disabilities, acquired brain injury, and mental health issues in a community-based setting.

Key Responsibilities:

  • Administer medications and treatments following established protocols and regulations.
  • Complete daily written notes, data tracking, and skilled assessments to ensure accurate documentation.
  • Communicate health changes in a timely manner to ensure seamless care coordination.
  • Utilize Positive Behavior Supports (PBS) practices to create a supportive work environment.
  • Provide physical support, assistance with lifting and transferring individuals as needed.

Requirements:

  • Possession of a Massachusetts license as a nurse.
  • 2 years of experience working with individuals with acquired brain injury or similar disabilities preferred.
  • Basic computer literacy, including data recording and electronic time and attendance software.
