About the Role:

We are seeking a skilled Physical Therapist to join our Home Health team at AccentCare. As a Physical Therapist, you will play a vital role in providing high-quality patient care and improving the lives of our patients.

Key Responsibilities:

  • Conduct thorough assessments and develop personalized treatment plans for patients in their homes
  • Provide one-on-one therapy sessions to patients, focusing on improving mobility, strength, and function
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination
  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to treatment plans
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Master's degree in Physical Therapy from an accredited program
  • Current licensure to practice as a Physical Therapist in the state of operation
  • Minimum 9 months of experience as a Physical Therapist, with a preference for community or home health experience
  • Valid CPR certification
  • Ability to travel to all business locations
