Job Summary

We are looking for a compassionate and experienced Home Health RN to provide high-quality patient care in the comfort of patients' homes. As a travel nurse with Fusion Medical Staffing-Home Health, you will have the opportunity to work with a variety of patients and develop your skills in a fast-paced and challenging environment.

Responsibilities
  • Conduct thorough nursing assessments of patients and document findings in patient records
  • Communicate and collaborate with physicians and other healthcare team members to ensure seamless patient care
  • Provide and coordinate patient care with other healthcare team members, including physical therapists, occupational therapists, and social workers
  • Monitor patients' responses to interventions and report outcomes to healthcare team members
  • Administer medications as ordered and document administration
  • Provide education to patients and families on disease management, medication management, and other health-related topics
  • Participate in on-call rotations and respond to patient emergencies within 30 minutes
Requirements
  • One year of experience as a Registered Nurse, with at least six months of recent Home Health experience
  • Current RN license and BLS certification
  • Valid, unrestricted driver's license and valid auto insurance
  • Ability to work in a fast-paced environment and prioritize multiple tasks
  • Excellent communication and interpersonal skills
