Job Title: RN:Home Care Case Mgmt

American Medical Staffing is seeking a skilled RN to join our team as a Home Care Case Manager. As a key member of our team, you will be responsible for providing high-quality care to patients in their homes, promoting independence and optimal health outcomes.

About the Role:
  • Provide comprehensive nursing care to patients in their homes, including medication management, wound care, and infusion therapy.
  • Conduct thorough assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
  • Monitor patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes.
  • Provide emotional support and counseling to patients and families coping with chronic illness or end-of-life care.
Requirements:
  • Valid RN license and willingness to obtain MD licensure.
  • Experience working in the home setting and using Electronic Medical Records.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
What We Offer:
  • Competitive pay rate: $2,160-$2,360 gross per week.
  • Flexible schedule: Monday-Friday, 40 hours weekly.
  • Start date: 10/28/2024.
  • Located in Hagerstown, MD.
