PACE Program Eligibility Nurse

High Desert PACE is seeking a highly skilled and compassionate PACE Program Eligibility Nurse to join our team. As a key member of our outreach and enrollment team, you will play a critical role in determining the eligibility of future participants for our program.
Key Responsibilities- You will work closely with our Business Development Representatives and Enrollment Representatives to assess the level of care required by prospective participants.
- Schedule and conduct home visits and/or center-based clinic appointments to make level of care assessments and determine eligibility.
- Collaborate with the interdisciplinary team (IDT) and outreach & enrollment (O&E) team to ensure that prospective participants receive the necessary care and support.
- An Associate degree from an accredited nursing school is required, along with a current Registered Nurses License in the state where working and 3 years of healthcare experience with an emphasis in geriatrics.
- The ideal candidate will possess strong organizational skills, effective listening and communication skills, and the ability to manage changing priorities.
- A valid driver's license and use of personal vehicle are also required.
$95,000 - $130,000 per year, depending on experience.
