We are seeking a skilled Home Health Care Coordinator to join our team at Amedisys. As a key member of our care coordination team, you will be responsible for educating healthcare professionals, patients, and families about home care services, developing and maintaining relationships with key referral sources, and coordinating patient admissions.

Responsibilities
  • Educate health care professionals, patients, and families about home care services
  • Develop and maintain relationships with key referral sources
  • Coordinate patient admissions with the team
  • Conduct on-site visits to assess patient needs
Compensation Package

$75,000 per year plus benefits package

Qualifications

The ideal candidate will have:

  • At least one year of experience in a healthcare related business development/outreach role
  • Current RN or LPN license, specific to the state(s) you are assigned to work
  • Reliable transportation and current driver's license
