About the Role

Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse - LVN/LPN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning from hospital to home and providing continuous care to ensure their well-being.

Responsibilities
  • Implement physicians' prescribed plans of care, ensuring timely and effective treatment.
  • Conduct patient assessments and coordinate care with our multidisciplinary team.
  • Administer prescribed medication, treatments, and therapies, maintaining accurate records of care provided.
  • Provide health education and training to family members, empowering them to support their loved ones.
  • Collaborate with our Case Manager to ensure seamless care transitions and address any concerns or changes in patient condition.
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within last 12 months).
  • At least six months of prior hands-on nursing experience, with a strong commitment to delivering exceptional patient care.
