Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager in the Washington, IA area. As a Case Manager, you will be responsible for providing comprehensive care to our patients in the comfort of their own homes. This is a full-time opportunity with a competitive salary range and a comprehensive benefits package.
Responsibilities- Provide comprehensive care to patients in their homes, including assessments, care planning, and implementation of care plans.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Conduct regular home visits to assess patient needs and provide ongoing care and support.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and effective care.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Active Iowa RN licensure.
- CPR certification.
- Valid driver's license, insurance, and reliable transportation.
- Prior home care experience preferred, but not required.
- Experience with OASIS, Medicare/Medicaid regulations, and EMR software preferred, but not required.
- Ability to work independently with good time management and organizational skills, as well as part of a close supportive team.
