Job Summary

The LPN Post Acute LTC will partner with our residents and other care providers to create a safe and comfortable therapeutic environment by providing direct and indirect care.

Unit Overview

The LPN will work on one of our three units:

  • 3 East: A 31-bed specialized unit for residents with Alzheimer's/dementia, providing tailored care to meet their needs.
  • Transitional Rehabilitation Unit: A 42-bed unit providing short-term, skilled nursing and rehab services to patients recovering from fractures, joint replacements, back surgery, stroke, and other illnesses or injuries.
  • 3 West: A 42-bed long-term care unit providing needed care to residents requiring long-term placement in a home-like environment.
Responsibilities

The LPN will be responsible for:

  • Providing direct and indirect care to residents, including IV therapy, wound care, and geriatric assessment and management.
  • Working collaboratively with the RN and other care providers to create a safe and comfortable therapeutic environment.
  • Utilizing verbal and non-verbal communication techniques to understand and address the needs of residents with Alzheimer's/dementia.
  • Participating in unit-based quality improvement initiatives to enhance resident care and satisfaction.
Requirements

To be successful in this role, the LPN must have:

  • A valid State of Maine or compact Licensed Practical Nurse license.
  • 1-3 years of previous RN/LPN experience, preferably in a post-acute care setting.
  • Strong teamwork and communication skills, with the ability to work well within a team of other care providers.
  • Flexibility to cover 24 hours/day, 7 days per week, and float to other units as needed.
