We are seeking an LPN Care Coordinator to join our team at Prisma Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and promoting healthy outcomes.

About the Role:

The LPN Care Coordinator will be responsible for providing routine, standardized nursing care for patients under the supervision of the physician. This includes assisting physicians with examinations, procedures, and other activities related to direct patient care. You will also arrange for patient appointments, monitor and maintain stock of medical supplies, and ensure that all required records are in the chart.

Key Responsibilities:

  • Assist physicians with examinations, procedures, and other activities related to direct patient care.
  • Arrange for patient appointments for diagnostic testing, consults, referrals, and admission to hospital.
  • Monitor and maintain stock of medical supplies and proper functioning of equipment.
  • Document patient's vital statistics and other encounter data into patient chart and ensures that all required records are in the chart.
  • Obtain brief medical history and chief complaint from patients.
  • Call in prescriptions for patients according to physician order.
  • Promote positive health behaviors and self-care skills through patient education.
  • Prepare exam rooms with appropriate instruments, supplies, and equipment.
  • Clean exam rooms between procedures and exams.
  • Receive and return clinically related telephone calls; screening and forwarding those requiring physician action.
  • Complete medical/physical history forms and other related information for insurance companies.
  • Inform physician of changes in schedule.

Supervisory/Management Responsibility:

This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements:

Completion of a LPN program recognized by the SC State Board of Nursing.

No experience required.

Required Licensure/Registrations/Certifications:

Licensed to practice as a LPN in South Carolina.

Earning Potential:

The estimated salary for this role is $55,000 - $65,000 per year, based on location and experience. However, actual compensation may vary depending on individual qualifications and circumstances.
