Overview:
A nonprofit organization represented by Insight Global is seeking a Client Support Specialist.

The Client Support Specialist will assist both children and adults (ages 3+) in crafting plans that ensure the needs and preferences of the clients are met while enhancing their life opportunities.


The Client Support Specialist plays a crucial role in helping clients select services and supports, coordinating the delivery of these services, and overseeing their implementation.

Ideally, the Client Support Specialist will possess a solid background in providing support coordination or services to individuals with intellectual or developmental disabilities.

Key responsibilities include conducting regular on-site visits to clients' residences to ensure safety standards are upheld, scheduling appointments, traveling within the community, and aiding in the identification and development of community resources to support the client's well-being at home and in the community. The role also involves completing assessments to determine each client's needs and desires, gathering essential information to assist clients in articulating their needs and preferences, and helping them formulate their Individual Support Plan (ISP) and personal budget. The Client Support Specialist will update the ISP as necessary and at least once a year, support clients in identifying their desired outcomes, and ensure that the ISP addresses these goals. Additionally, the specialist will identify appropriate services to meet client needs, complete referrals, coordinate the services being received, and address any obstacles to service delivery by being knowledgeable about a comprehensive range of options and resources.

The Client Support Specialist must be adept in computer usage, possess proof of COVID vaccination, and have reliable transportation.



Responsibilities:

  • Conduct assessments to identify each client's needs and preferences, utilizing both formal and informal tools to gather necessary information.
  • Assist clients in developing their Individual Support Plan and personal budget, ensuring updates are made as needed and at least annually, with a person-centered approach.
  • Support clients in identifying their desired outcomes and ensure that the ISP addresses these effectively.
  • Identify services that will fulfill client needs, complete referrals, and coordinate the services being provided. Resolve any barriers to service delivery by being well-informed about available options.
  • Monitor services to ensure they are delivered as planned and that identified outcomes are being achieved, while also ensuring client safety and health.
  • Help in identifying and maintaining the client's support network, including family, friends, and associates.
  • Assist in arranging and managing generic resources and supports effectively.
  • Identify and develop community resources to enhance the client's well-being at home and in the community.
  • Stay informed about relevant publications and policies, including The Everyday Lives Publications and Quality Progressions.
  • Participate in training to remain knowledgeable about current best practices in supporting individuals with intellectual disabilities.
  • Educate families and clients about their rights and due process.
  • Build a professional relationship with families and clients on the caseload, ensuring a deep understanding of each individual supported.
  • Maintain a customer service focus by meeting with clients and their families at their convenience.
  • Complete all other duties necessary to support clients and their families, as well as adhere to Quality Progressions.
  • Provide oversight and assistance with FDSS grants.
  • Demonstrate initiative and leadership in all tasks.

Required Qualifications:

  • Valid Driver's License
  • College degree with a transcript showing at least 12 credits in Social Science-related coursework
  • Reliable vehicle for community travel
Proof of COVID vaccination is required.

  • Preferred experience of 6 months to 1 year
Preferred Skills:

  • Bilingual or knowledge of sign language
