Job Summary and Qualifications

As a Clinical Nurse Case Manager, your responsibilities will include promoting patient-centered care by coordinating the care plan for patient stays, managing lengths of stay, ensuring effective resource management, and developing safe and appropriate discharge plans in collaboration with the multidisciplinary team.

You will facilitate the progression and transition of care using established criteria alongside the multidisciplinary team.

Your role will involve coordinating activities that enhance quality outcomes and patient throughput while maintaining a balance of optimal care and resource utilization.

You will provide case management services for both inpatient and observation patients as assigned.

Your responsibilities will include identifying patients at risk for adverse outcomes during transitions between care levels/settings.

You will conduct comprehensive assessments of psychosocial, medical, and discharge needs of patients and families, as well as evaluate available resources.

You will reassess patients' clinical conditions as necessary.

Consideration of patients' readmission status or risk of readmission will be crucial, and you will develop strategies to mitigate these risks, including education on accessing healthcare resources, preventative education, and community-based resources.

You will coordinate care plans and drive discharge planning by collaborating with the multidisciplinary healthcare team, particularly with the patient's physician, to ensure successful care transitions.

You will partner with Social Services to confirm that post-acute medical needs and levels of care are appropriate.
Experience

- Minimum of 2 years' experience in case management or 3 years' experience in clinical nursing.
- InterQual experience is preferred.
Education

- Associate Degree in Nursing or Nursing Diploma.
- Bachelor's Degree in Nursing is preferred.
Licenses and Certifications

- Current licensure as a Registered Nurse in the state(s) of practice according to law and regulation.
- Certification in Case Management is preferred.
Schedule

- 32 hours, rotating weekends (1 weekend every 3 weeks).
