As a rapidly expanding Medicare-certified home health organization, we prioritize the well-being of both our employees and the individuals we serve.

Key Responsibilities:

  • Delivering personal care services tailored to the needs of patients.
  • Monitoring patient conditions and reporting any significant changes to the supervising Registered Nurse.
  • Assisting with daily living activities, including bathing, grooming, and meal support.
  • Maintaining a clean and safe environment for patients.
  • Documenting care provided and observations made during visits.

Qualifications:

  • Completion of a state-approved training program for Certified Nursing Assistants.
  • A minimum of one year of experience in a similar role, preferably in home health or hospice settings.
  • Strong communication skills and the ability to follow written instructions.
  • Empathy and compassion for patients with diverse needs.
  • Current CPR certification and a valid driver's license.

Working Conditions:
