We are seeking a skilled Wound Care RN to join our team at Harlem Center for Nursing and Rehabilitation in New York, NY.

Job Summary:

The successful candidate will be responsible for managing preventative wound programs and wound treatment programs, completing wound analysis reports, and providing education on wound management and prevention.

Key Responsibilities:
  • Develop and implement wound care plans to promote healing and prevent further complications
  • Conduct regular wound assessments and document findings
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Provide education to residents and families on wound management and prevention
  • Administer prescribed medications and procedures as necessary
  • Monitor resident progress and adjust care plans as needed
Requirements:
  • Valid NYS RN License
  • 1+ Year of nursing experience, preferably in long-term care
  • Wound care certification preferred
  • Point Click Care experience a plus
