About Finger Lakes Community Health:

Finger Lakes Community Health (FLCH) was founded in 1989 with a mission to serve the region's agricultural workers by linking them to area resources and providing essential health services. Today, FLCH has grown to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities, and backgrounds.

Job Summary:

The Healthcare Navigator will be responsible for connecting underserved populations to healthcare services, empowering individuals to take control of their health, and building strong partnerships with local organizations to strengthen services. This role requires excellent communication skills, cultural competence, and the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Connect individuals and families to healthcare services, including medical, dental, and behavioral health care.
  • Empower patients to prioritize their healthcare needs and make informed decisions about their care.
  • Assist patients in accessing available resources, such as social support and informal counseling.
  • Make referrals to healthcare services and track outcomes.
  • Participate in clinical outreach initiatives to meet the basic health needs of underserved populations.
  • Establish partnerships with local organizations to build strong relationships and improve services.
  • Provide information, education, and support to patients in their communities.

Requirements:

  • High school diploma and experience working with underserved populations.
  • Fluent bilingual skills in English and Spanish.
  • Basic computer skills and proficiency in electronic health records.
  • Clean driving record and valid New York State driver's license.

Physical Requirements:

  • Able to lift up to 50 pounds and work in outdoor conditions.
  • OSHA level 1 certification.
