Job Summary

We are seeking a dedicated and compassionate Direct Care Worker to join our team at Change Incorporated in DARIEN, CT.

Job Responsibilities
  • Provide supportive services to individuals in a home and community-based setting, including elder care, mental health support, and in-home assistance for individuals with developmental disabilities or acquired brain injuries.
  • Establish and maintain a helpful and supportive relationship with the participant through effective communication, teaching, and assisting with daily living tasks.
  • Accompany clients to medical appointments, social activities, and other events as outlined in their recovery plan.
  • Perform light housekeeping duties and assist with daily tasks.
  • Use reliable transportation to transport clients as needed.
Requirements
  • Experience in the healthcare field providing direct care or strong customer service skills.
  • Reliable transportation, valid driver's license, and current automobile insurance.
  • Ability to lift 15 pounds.
