Role Responsibilities:
The Advanced Practice Provider (APP) is responsible for:
- Providing comprehensive medical management for a designated group of nursing home residents.
- Conducting thorough assessments and developing tailored care plans in collaboration with the primary care team.
- Engaging with residents' families to ensure clear communication regarding care goals and needs.
- Utilizing clinical guidelines to inform treatment decisions and maintaining accurate documentation of health and functional status.
- Participating in care team meetings to discuss member progress and adjustments to care plans.
Key Duties:
The APP will:
- Deliver collaborative primary care services, ensuring that treatment aligns with members' individual care goals.
- Implement the Longevity Health Model of Care, focusing on in-place treatment and advanced care planning.
- Manage clinical operations such as prior authorizations and quality gap closures, contributing to overall membership growth.
- Act as a trusted resource for members, families, and nursing home staff, advocating for members' needs and facilitating timely interventions.
- Maintain effective communication with members' primary care providers and care teams regarding treatment plans and changes.
Qualifications:
Applicants must possess:
- A valid state license as an Advanced Practice Nurse or Physician Assistant.
- National board certification as a Nurse Practitioner or Physician Assistant.
- Experience in geriatric or hospice care is preferred.
- A valid DEA license and active provider numbers for Medicare.
Education:
A Master's degree in Nursing or a related field from an accredited institution is required.
Experience:
Minimum of six months of experience in a clinical setting, preferably with a focus on geriatric care. Strong communication skills and the ability to work independently and as part of a team are essential.
