Job Summary

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the home setting. As a Pediatric Home Health Care Nurse, you will be responsible for delivering skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.

Key Responsibilities
  • Provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting care provided in a timely manner.
  • Administer prescribed medication, treatments, and therapies, while maintaining accurate records and adhering to infection control measures.
  • Conduct patient assessments, coordinate care, and educate patients and family members on disease processes, self-care techniques, and prevention strategies.
  • Participate in the development and implementation of individualized care plans, ensuring patient-centered care and achieving expected outcomes.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
Work Environment
  • Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
