Job Overview

We are seeking a Registered Nurse - On Call to join our hospice team. This is a full-time weekend on-call position, Friday - Sunday 8am-8pm. As a member of our team, you will report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.

Responsibilities
  • Act as primary liaison between physician, patient/family, and hospice team
  • Provide direct patient care services to hospice patients and supportive care to the patient's family after normal business hours on an on-call basis
  • Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care
  • Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs
About You

We are looking for a compassionate, kind, and empathetic individual who is a current RN license holder (in the state of requested employment or ability to obtain) with 1+ years of nursing experience in hospice, home health, or oncology.
