POSITION SUMMARY
The role of the Certified Nurse Assistant is to deliver personal care services to patients in the inpatient unit, under the supervision and guidance of the designated staff nurse. This position adheres to the Nurse Practice Act along with the policies and procedures of the agency.

KEY RESPONSIBILITIES
The following outlines the essential duties and responsibilities expected in this role:

Personal Care Services
A. Provide personal care services as outlined in the Plan of Care, which may include:
1. Assistance with bathing (tub, shower, bed bath), oral hygiene, denture care, skin care, grooming (shampooing hair, shaving), and support with bathroom needs (bedpan, bedside commode).
2. Dressing assistance, transferring, and ambulation support.
3. Maintain a clean and safe environment for patients.
4. Administer therapeutic exercises as directed and supervised by nursing staff.
5. Perform simple non-sterile wound care under the guidance of the staff nurse.
6. Assist with changing colostomy bags and g-tube dressings.
7. Support patients in meeting their nutritional and hydration needs, including feeding and accurately recording intake.
8. Provide care for indwelling and suprapubic catheters.

B. Accurately measure and document vital signs including oral, rectal, and auxiliary temperatures; pulse; respiration; and blood pressure.
C. Promptly report significant changes in the patient’s condition to the staff nurse for appropriate intervention.
D. Offer respite care for patients, families, or caregivers.
E. Reinforce previous teachings provided by the RN.
F. Assist with post-mortem care as needed.
G. Participate in weekend and holiday rotations, and be available for standby and weekend calls as required.
H. Perform additional duties as assigned.

ADMINISTRATIVE DUTIES
A. Ensure timely and accurate documentation of home visits.
B. Engage in the timely revision of patient care plans.
C. Adhere to agency policies, procedures, and mission statements.
D. Execute tasks as assigned by the Staff Nurse and Director of Clinical Operations.
E. Maintain a professional appearance in accordance with dress code policies.
F. Exhibit a consistently positive attitude.
G. Demonstrate knowledge of and compliance with Hospice policies and procedures.

COMMUNICATION SKILLS
A. Establish and maintain effective communication with team members and other departments.
B. Listen and respond to patient and family needs in a sensitive and timely manner, keeping all parties informed of developments.
C. Maintain a cooperative demeanor towards patients, families, and all team members.

PROFESSIONAL DEVELOPMENT
A. Attend and participate in team meetings, patient care conferences, and bereavement conferences.
B. Complete required in-service programs and maintain ongoing education as necessary.
C. Participate in orientation for new employees, volunteers, and students as assigned.

QUALIFICATIONS
Certification as a Nurse Aide by the Texas Department of Health is required. A personal vehicle for transportation during work hours and access to a personal telephone are necessary. Candidates must possess a valid driver’s license and insurance as mandated by the State of Texas. Proficiency in reading, writing, and speaking English is essential for effective communication and job performance.

WORKING CONDITIONS
This position involves working in a hospice inpatient unit and may require travel to various locations, including patients' homes and assisted living facilities. The role is subject to frequent interruptions and involves interaction with patients, families, and healthcare personnel under various conditions.

PHYSICAL REQUIREMENTS
The candidate must be physically fit and capable of working in diverse settings, including patients' homes and healthcare facilities. The role requires the ability to sit, bend, lift, and walk intermittently, with the capacity to reposition a minimum of 50 pounds.
